You MUST complete this loan request form to activate your Claremont McKenna College Permanently Restricted Loan.

This is NOT a promissory Note.

Each year you must complete and submit a new request form to our office. If you did not borrow from this loan program previously, you must complete and submit a CMC Loan Permanently Restricted Master Promissory Note.
