Federal Perkins Loan Program

For Returning Perkins Borrowers:

Complete and return the Perkins Loan Rights and Responsibilities Form to the Office of Financial Aid to complete the process.

For First Time Perkins Borrowers:

Step 1:

Please review the CMC Federal Perkins Loan Fact Sheet for guidance.

Step 2:

Complete and return the Federal Perkins Loan Master Promissory Note to the Office of Financial Aid.

Step 3:

Complete and return the Perkins Loan Rights and Responsibilities Form to the Office of Financial Aid.

Step 4:

Complete and return the Personal Information Form (PIF) to the Office of Financial Aid.

Exit Counseling for all Perkins Loan Borrowers:

If you are graduating, taking a leave of absence or enrolling less than half-time, federal regulations require you to complete an exit interview counseling session with the student loan specialist at:
